Task 1: Written questions The student must appear in an open book test and correctly answer all 12 questions to show that he/she understands the vocational knowledge required of this unit.
Task 2: Case studies The student must read two case studies and complete the questions for each.
Task 3: Roleplays The student has to undertake two role plays.
Task 4: Behaviour management plan The student must chart out a behaviour management and review plan for the client from Roleplay 2 in Assessment Task3.
Task 5: Project – dementia diseases Studentmust complete a research project on varied types of dementia diseases.
Task 6: Project – working with clients with dementia The student will be required to monitor two clients with dementia for three days and prepare and complete a journal for each client. This would include describing the client’s conditions, symptoms, family’s opinion, practices to which client responded well and other observations.

Once the student completes all six assigned tasks the Assessor will assess the performance of the student in all tasks and provide suitable feedback.

CHCDIS002 Follow Established Person-Centredbehaviour Supports

Under this module, the student must complete four assessment tasks to demonstrate that he/she is adequately competent to vocationally provide established person-centered behaviour support services. The four assessment tasks are as follows:

Assessment Task Description
Task 1: Written questions The student must correctly answer all 25 questions in an open book test to show that he/she understands the knowledge required for established person-centered behaviour support services.
Task 2: Roleplay The student must perform a role play using a simulated video on self-injury as guidance. The student must respond to a critical incident involving a behavioural issue of a person with a disability and answer questions on effective response strategies.

The Assessor will evaluate how well the student responded to a critical incident involving adverse behaviour in a simulated environment, using appropriate support strategies and communication.

 

Task 3: a case study The student must assess and review a sample behavioural support plan devised by the Department of Human Services and answer questions based on the case study.
Task 4: Journal The student must record and prepare journal reports for three different clients in your work placement service. The student must document the behaviour management observations and support you have provided to three clients in your work placement service over least three different occasions

Support Relationships with Careers and Families

Under this module, the student must complete four assessment tasks to demonstrate that he/she is adequately competent to vocationally provide support to people who want to manage their relationships with careers and family. The four tasks are as follows:

Assessment Task Description
Task 1: Written questions The student must correctly answer all 17 questions in an open book test to show that he/she understands the knowledge required for providing support services to the client to help them manage their relationships with career and family.
Task 2: Case studies The student must read three case studies and complete the questions for each.
Task 3: Workplace project 1 There are two parts to this task:

§ Part A: The student must discuss strength-based solutions with two co-workers during your work placement service

§ Part B: The student must answer a set of questions based on what he/she learned during Part A exercise.

Task 4: Workplace project 2 The student must evaluate an existing client care support plan (from a client at your work placement service) and complete an assessment of the roles and responsibilities of those in the client’s support team. This would involve examining role and responsibility of each member in client support team, rapport of each member with the client, qualities required to perform each role in the team, nature of support services provided to the client, impact on clients etc. The Project report must describe how each member contributes to the design and delivery of support services provided by the support team.

CHCCCS011 Meet Personal Support Needs

Under this module, the student is required to complete four assessment tasks to demonstrate that they are adequately competent to vocationally provide personal support needs of the client. The four assessment tasks are as follows:

Assessment Task Description
Task 1: Written questions The student must correctly answer all 11 questions in an open book test to show that he/ she understands the knowledge required to provide personal support needs of the client.
Task 2: a Case study The student must read the case study and complete the questions.
Task 3: Roleplays The student must work in a simulated workplace environment. He/she will be evaluated:

§ transferring a person in and out of a vehicle

§ transferring a client from a bed to a chair using a hoist

§ using a hoist in a falls recovery situation.

Task 4: Observation task The studentwill be assessed in his/her work placement service supporting at least two clients with a variety of personal care needs.

CHCHCS001 Provide Home and Community Support Services

Under this module, the student is required to complete six assessment tasks to demonstrate that they are adequately competent to vocationally provide home and community support services. The four assessment tasks are as follows:

 

Assessment Task Description
Task 1: Written questions The student must correctly answer all 12 questions in an open book test to show that he/she understands the knowledge required of for home and community support services.
Task 2: Case studies The student must read three case studies and complete the corresponding questions.
Task 3: Observation task There are three parts to this task:

§ Part A: Student must organise for two home visits, including discussing policies and procedures with the Assessor.

§ Part B: Student will be assessed for working with two clients in a home support set-up.

§ Part C: Student must answer a set of verbal questions about the clients and his/her home visits.

Before commencing the abovementioned tasks, the students need to refer to the Student Assessment Booklet and fill an Assessment Task Cover Sheet and sign the student declaration and submit it to the Assessor.

Once the student completes all the assigned tasks under these five modules, he/ she can submit them to the Assessor. The Assessor will assess the performance of the student in all tasks and provide suitable feedback.
