Transitioning to Practice

A patient named Josie who was 18 months -old was admitted to the hospital. After a few days, it was found that she died due to dehydration and wrong administration of narcotics. The registered nurses did not monitor the deteriorating conditions of the patient. I felt shocked and deeply distressed about the attitude of the nurse that resulted in patient death. I felt that the nurse being a central part of the health care team should have performed their responsibilities and duties with full attention to prevent harm to the patient’s safety. The worst part of the situation was the negative attitude of the nurses that lead to the death of the patient. However, the only positive part was that the mother of that patient was involved in the health care treatment of her daughter as she was informed during the process of shared decision making because the patient herself was very small to make decisions on her own.

It is learned that to provide comprehensive care, continued care, monitoring of health conditions and evaluation of outcomes are the prime responsibilities of a nurse to ensure patient safety. It is also required to make nurses aware of the importance of following the NMBA and NSQHS standards to provide patient-centered care and ensure patient safety. The consumers like patients or the family should be encouraged to get involved in their decisions of treatment plans, health care service evaluation, and delivery (National Standard and Quality Health Service Standards, 2020). The health care system should be designed to support patients and families as well. The nurses should evaluate the risks or hazards from the patient’s health outcomes and keep a track of records to provide patient-centered care.

The nurse and other health professionals should communicate with each other effectively to ensure teamwork and effective care for the patient with ensured patient safety. Every decision, measurement, or evaluation should be discussed with the patient or the family members to support the patient with shred decision-making strategies. According to the National Standard and Quality Health Service Standards (2020), the nurses should document the patient’s history, drugs administered, and record of health status. They should review the information and take feedback as well. They should involve the families in decision making and provide patient-centered care by meeting the needs of the patient. According to the Nursing and Midwifery Board (2020), the patient or his family members should be involved in the design and development of the health care plans. They should also conduct integrated risk assessments or identifications to manage the health care plan accordingly. Moreover, they should be trained to follow the NMBA and NSQHS standards which help in providing quality, effective, and patient-centered care with patient safety. Professional responsibility and accountability of a nurse is such that it should be encouraged by application of policy, training and procedures such that the practice of the nurses can be improved and effective can be provided.
