Consideration of Communication Processes Between Nurse, Patient and Family

Good morning all,

Today will be discussing on one of my encounter with the patient, while managing care for him. The patient is a 91 year old man who recently lost his wife and was moved to the aged care facility to help him with his activities of daily living. The patient was not independent enough in managing his tasks on his own. The given case study is a reflection as to how gap of effective communication between me and the patient, led to his unrest. The presentation will be covering various aspects of verbal as well as non-verbal communication in relation with the case study. Use of SOAP note will be made to evaluate the patient and the SBAR approach of clinical handover will be followed to give the hand-over of the situation to the senior subordinates.

There are mainly two types of communications. One is verbal and the other is non-verbal. The act of verbal communication involves transferring and sharing of information with one another. Whereas, the nonverbal method of communication mainly helps in sharing of thoughts and ideas through the means of facial expressions, body languages and so on. The means of both verbal as well as non-verbal communication is important in this given case scenario.

Role of verbal communication-

There nurse practioner was verbal enough to appreciate the patient about the progress he was making. The patient is of an Italian descent, therefore, he might not be able to grasp the information provided to him. The role of a translator or mediator is also very important in the given scenario. It will be useful in facilitating true information to the patient. Appreciating the patient can be deemed as an effective method of verbal communication. This can be helpful in motivating the patient the patient to do better. The vocal directions given to the patient can be helpful in providing him with a purposeful intervention.

Verbal communication also helps in maintaining a command over the process and thus, useful for attaining improved healthcare outcomes from the patient. The nurse professional can also be observed to have a stern stand on his statement. He can be observed to be clarifying the doubts of the patient and rectifying his statement. This can be reflected as a good stance on the nurse’s part in standing correct and stern on his statement.

Role of non-verbal communication-

The care nurse was being supportive and humble to the patient. He was able to provide him with the required care and support throughout the process. Although the conversation lacked empathy on my part. Given the fact that the patient lost his wife recently, he is very depressed. He can be counselled or talked to, before commencing with any interventions. This will be helpful in analyzing for the details of the patient beforehand and will be able to draft an intervention plan in accordance with the same. Empathy is an integral part of the nursing care. It is useful for connecting with the patient on deeper level.

It is helpful for the nurses as well, as it enables to provide and insight to the nursing professional about the mental state of the patient. The nurse did not include the patient’s family in the process. Indulging the family in the care and coordination of the patient can be an effective method to connect with the patient on a personal level. The body language of the nurse was humble and he did not respond back violently to the patient’s son. It is crucial from the point of view of averting any conflict that might arise from the situation. His body language was although not clear while appreciating the patient that led to the development of the confusion.

Reflection on resilience and self-care in communication in nursing-

It is an important an integral part of the nursing care and management. This element of communication in nursing care is important for promoting the character of empathy and compassion in nursing professionals. The resilience in nursing care is also important for improving the quality of care of nursing provided to the patient. It is also helpful in sustaining the clinical practices in their full capacity, to be available to be used in patient care. It is also helpful for the nursing professional to cope up with the work stress and maintain their composure while managing care for the patient. This is an effective method in our given case study as the patient’s son is being violent to the nurse and the nurse is still able to maintain his calmness. He is also been able to handle the situation in a methodical manner.

SOAP note-

It is an effective assessment tool that can be helpful for the nursing professionals to gather information about the patient and set a clinical diagnosis for him. This is also a helpful method in drafting a treatment intervention plan for the patient.

S stands for subjective observation. The subjective observation in the given case study includes for the fact that the patient is unsafe for self-care and has a reduced mobility status. He is also a recent widower and is depressed with the loss of his wife.

O stands for objective observation. The objective observation of the patient includes his history of diabetes, hypertension and osteoporosis. For all of these conditions he is on medical management. He is also having pain at different body locations, especially at knee joint due to osteoporosis.

A stands for assessment. The main points of assessment in the given case study includes, patient having a reduced ability of self-care management, depression, lack of mobility, lack of independence status in performing the activities of daily living.

P stand for planning the care and management for the patient. It also includes, recognizing the need for any further diagnostic interventions required for the patient and planning a well-structured discharge for them. Planning for this patient will be inclusive of promoting patient and family education, providing counselling for the treatment options of the patient and also for his depressive state and most importantly promoting an independent state of living for the patient.

SABR tool-

Situation- The case is about Mr. Rossi, who was appreciated for his progress made regarding his healthcare and was mistaken by him, as him being getting discharged from the care facility.

Background- Patient has multiple comorbidities and he lacked the capability of performing the tasks on his own. He was also unable to manage his self-care and was therefore, shifted to a care facility equipped with the professional support required by the patient.

Assessment- Patient is in a depressive state and wants to get back to his house. He lost his wife 7 months back and due to this confusion of communication gap, he is furious about him not getting discharged for the care facility.

Recommendation- Am effective and peaceful communication should be established between the patient and his family in order to help them analyze the criticality associated with the patient present clinical condition. It will be helpful in providing the family with an insight as to how many risk factors the patient is subjected to, due to his lack of ability of self-care and management.

Written communication is important to keep a close track on the turn of events of the episode. This will be helpful in resolving any conflict related to the ethical and legal concerns that might arise from the present situation. This will also be helpful in identifying for key factors that might be useful in planning future care and interventions for the patient.

Verbal communication is crucial to clear all of the myths and concerns of the patient as well as his family. It is also important from the point of view of providing the patient and his family with a sense of re-assurance and will thus, be helpful in calming the patient. Strong and robust verbal communications channels will be helpful in ensuring smooth facilitation of delivery of healthcare services to the patient with an ease and required efficacy.
