Mental State Examination

General appearance	Peter is 38 years old Australian man who has spent his life both in Sydney and Queensland. Peter appears to be physically aggressive and verbalizes abnormal content during the conversation. Also, he seems to be disturbed and not self-caring, voices to himself and voices potential violence towards other people. Peter engages himself in social drinking.
Behaviour	Peter engages in aggressive conversations and possesses abnormal behaviours, such as altering bizarre content to other men. Occasionally Peter has been displaying poor relationships with other staff males. He holds the belief that all males are paedophiles. His behaviour also makes him rarely attend work, and is later dismissed. Peter does not admit he has mental problems and declines getting medication.
Speech	Peter aggressively raise his voice while in conversation with the other casual workers, family members, police, and other men in society. He talks to himself, an aspect that makes his parents know that he out of his mind.
Mood and Affect	Peter feels happier when engaged in a conversation that does not involve him being admitted to the hospital. His mood drastically changed when cautioned about his queer behaviour and asked to go for a mental check-up.
Thought process	Peter has no logical flow of thoughts, and this gives evidence that he thinks abnormally.
Thought content	Peter beliefs that he is not abnormal, and therefore he flees from home to Queensland. He again flees from the hospital to Sydney after been required to undertake mental health medication. He believes that his uncle is a problem for their family, and most men are paedophiles, including his brother-in-law. He beliefs that he is not mentally challenged and been hospitalized will be insignificant.
Perception	Peter displays a disturbed body image.
Cognition	Peter's cognition is challenged with a low concentration on life issues such as schooling and work, and therefore decides to quit school in grade 11. Also, his lower concentration made him been chased away from work.
Insight	Peter possesses an impaired insight as he does not acknowledge that he has cognitive behaviour, and to correct the condition, he must seek mental medication.
Judgment	Peter displays impaired judgment. He judges that all men are paedophiles. He also judges that he is mentally fit, and anyone thinking mentally impaired is wrong. He also judges that his uncle is a big problem for the family and assaults him.

SYMPTOM	INTERVENTIONS
1. Impaired judgment	Impaired judgment involves making conclusions that are not necessarily right but constitutes to peoples' opinion (Walsh, 2020). Peter judges himself to fit, which is wrong, according to his case, mentally. Peter is also impaired in his judgment that he perceives that all men he comes across are paedophiles, including his brother-in-law.
2. Abnormal verbalization	Abnormal verbalization is the deviation from what is perceived to be normal in the way people engage in conversation. (Tallon et al., 2020) peter quarrels with his family members, police, and wok mates to the extent of uttering bizarre content to them. Peter also shows abnormal verbalization in how he soundly talks to himself, which is also considered an abnormal condition (Grossnickle et al., 2016).
3. Physically aggressive	Physically aggressiveness is the act of attacking and confronting people with or without apparent reasons (Eraslan, 2019). Peter's physical aggressiveness is revealed when he quarrels with his parents, and they sent him to go and leave with his sister. At his sister's home, he shows aggressiveness again, and his sister sent him to hospital. Peter also portrayed his aggressiveness when he engaged with his workmate at the accommodation area, where he was reported to the police for action.

Nursing Report

Peter, who is 38 years, is admitted to acute mental health service for five days after been diagnosed with mental problems. This is the second admission after the first one that was initiated by the police when Peter caught quarreling with an accommodation mate. Peter's mental problem has been inherited from one generation to the other since his father Jim is known to have a mental breakdown that resulted from depression though it was not formally diagnosed. Peter's paternal grandfather committed suicide after having suffered from PSTD. Peter's grandfather had only one child Jim. Jim got married to Alice, and they were blessed with two children, Rebecca, who is 30 years, and Peter, who is 38 years old. Rebecca, who is Peter's sister, was diagnosed with anxiety.

Following the fact that Peter has a mental problem, he never admitted this fact, and therefore, he perceives the action of been hospitalized as a waste of time and resources. He perceives that his actions are normal and that anyone thinking of him been out of mind is mistaken.

Peter's physical appearance to be very aggressive, and often verbalizes abnormal and disrespectful content while conversing with other people. Also, he seems to be disturbed and not self-caring, voices to himself, and voices potential violence towards other people. Peter engages himself in social drinking.

Peter's judgment is impaired to the extent that he perceives issues with a significant deviation from the normal. Firstly, his judgment on his mental health is impaired since he thinks that his behaviours are normal and whoever thinks otherwise is wrong. Again, he wrongly judges the men he comes across as paedophiles. Peter also perceives his uncle as a threat and significant problem to the family and goes ahead to assault him.

According to pathology and diagnosis, Peter is diagnosed with schizophrenia for the first time he was taken to the mental health facility by the police. Peter fled from the facility within the first two months of admission and was nowhere to be found. Time later, it was noticed that he had traveled back to his matrimonial home to live with his family.

Peter was living his parents for six years, refusing to acknowledge that he had a mental illness, and he would not take the medicines prescribed by the medical officers. When they realised that Peter was abusing the prescribed medication, including Xanax, Diazepam, and social drinking.

Peter's case is worsening every day, and therefore proper medication will help manage his condition.
