Patient education, patient and carer participation and involvement in teaching, learning and assessment

Healthcare services are provided by professionals in respective institutions so that the people can be assisted to stay well. The services like Primary Healthcare services are provided at the entry point of the system with the coordination and person-focused approach to patients and families. Safer and accessible care is provided to achieve universal health coverage to prioritize a healthy lifestyle. During my learning period, I was taught to work hard and provide high-quality care to understand the consequences, causes, and possible solutions to patient problems.

Safer care is most important in healthcare so that the patient is not harmed because of unneeded hospitalizations, injury, disability, illness, and even death. We were taught in the class to work on implementation systems to improve safety at all levels of care. This approach includes patient engagement as an integral part of health care and patient-centred care. Based on my learning, I engaged patients in the respective healthcare institutions to make better-informed decisions about their care. Moreover, the alignment was made within the procedures to maintain the sustainability of health systems. The health services are taking more responsive and transparent means of providing services. Experts like general practitioners were also included to get engaged with patients and families for the decision-making process. Healthcare practitioners are majorly involved in multiple competitions and conflicts related to patient safety. This involved self-esteem, organizational, financial, and reputational priorities in healthcare as the divers to raise healthcare systems through patient engagement. Therefore, it is a good starting point in healthcare systems to promote understanding and accountability between healthcare providers and patients (Öztürk, et al., 2017).

I was placed in the healthcare institution in engaging patients with a dialogue regarding their health conditions, needs, personal values, and preferences. I found that the patients who were informed were likely to be more confident in reporting negative as well as positive experiences with mutual agreements. This helped in improving health outcomes, advancements in learning, and improvement by reducing adverse events. I found that patient and family engagement is equally important in the manner to provide care and treat patients. The patient experience and outcomes are the starting points for engaging patients. This information was collected through focused group discussions, surveys, interviews, or informal online feedbacks. The feedback provided details about patient experiences into needs, values, and preferences to improve the quality and safety of care.

Patient engagement is a promising area of patient involvement and patient education. It helped me in taking part in understanding patient perspectives and viewpoints so that the core values such as assurance, quality, compassion, and safety is given to patients. The exposure is collected from patient stories by providing valuable care and motivation for patient safety. In the healthcare institution, families, patients, and the staff was involved in the development and design of patient-centred care. Also, the recognition was gained from engaging the patient and family in policy development. During my duty, the patient and family were engaged in the dissemination and development of tools, education, and information materials. This contributed in designing research and planning to execute the plans. In my healthcare institution, we gave access to electronic health records so that patients could view their health records and get updated with treatment plans to increase treatment compliance.

In some cases, I recognized that patient and family engagement will begin with empowering and educating people to address their health needs. I use to involve the patients to encourage them in asking questions so that they could speak up their concerns and develop some tools to enhance their understanding of health issues. I identified that the factors that use to support patients' health relate to patients, health conditions, healthcare settings, healthcare professionals, and tasks. It means that demographic profile, primary care, knowledge, attitudes, abilities, behaviour, illness, and health literacy. These are said to be the hinderer of patient's perception and their role in health improvement. These challenges according to me should be overcome to improve education and communication given healthcare. The approaches that should be followed are providing education to patients and healthcare providers, obtaining feedback, and engagement for improvement. Engagement of patient and family is found to be an effective strategy in developing high-quality care, integration within the system, and people centeredness. From this, it was found that safety and quality improve patient care delivery.

According to policymakers and leaders, proactive commitment and engagement of patients and families are needed to implement the lessons learned. This upgrades direct care at an individual level, policy development, system design, and implementation through education, research, and regulations in the setting. This culture fosters transparency and openness to strengthen the patient-provider relationship. The healthcare providers and patients work with effective engagement in treating health conditions so that they get involved in decision-making (Weis, 2017).

Hence, from the above-discussed topic, we came to know that healthcare providers and policymakers in healthcare should ensure the involvement of patients and families in providing appropriate and up-to-date care. The records sharing with the patient and families help them in engaging by getting support and empowerment. The social and cultural norms should be taken into consideration so that the patient is treated acceptably and feasibly. The principles should be underpinned so that patients, families, care providers, and community partners are involved in providing care. The strategies such as education at both the organizational and individual level should be given to enhance patient education. The patient support must be given so that they are encouraged to support safety incidents, near misses, and safety concerns. Patient feedback systems should be promoted so that corrective actions could be taken on issues raised. Legislation support must be given to support families and patients to overcome issues related to safety. This helps in providing accurate, up-to-date, appropriate treatment in a user-friendly environment. The campaigns help in patient involvement, patient education, and patient engagement to strengthen patient safety in care provision. Adaption to these systems will help the communities to recognize the importance of positive interaction among families, patients, and healthcare providers (Thet, 2017).

Innovation and contemporary practice & development in teaching and learning (technology, simulation, games, experiential learning, distance, communities of practice, problem-based learning, etc.)

Education is the asset of a person that shows the right path to move forward. It is gained or provided to an individual not only the individual becomes educated but also adds rationale thinking, self-sufficiency, and knowledge. As I was pursuing nursing graduation, this required clinical knowledge, and skills to care for patients and families. This is done to provide the students with real-life exposure to handling situations with competency. The innovative strategies evolve diverse, complex, and ever-changing environment. Critical thinking in nursing profession provides a skilful, competent, and safe environment to professionals. During my course of nursing, clinical exposure was considered as the lifeblood of nursing education. My faculty and educators were having the responsibility to provide the most appropriate clinical knowledge to aid in the best learning of students.

My faculty felt that the critical thinking method should be followed because it not only helps the students in learning but also is equally important to know how the students learn. He said that the methods that were used earlier should be transformed so that improvements could be made in learning experiences. The strategies of teaching involve the experiences of "what is being done" and "with whom" to promote learning. This helps in developing creativity and innovation for both teachers and students. My teacher feels that any method that does not destroy the main purpose of teaching is the innovative method. This innovation will not only improve the education system but also will empower people to strengthen and galvanize the effort. The use of creative methods such as portfolios, web-based learning, case study solving, and other methods to enhance competencies using dictator methods. The innovation included grouping knowledge, attitude, and skills into new and different ideas.

The instruction in the classroom was given by demonstration, lecture, workshop, role-play, simulation laboratory, case-based learning, clinical simulation, mind mapping, blackboard learning, games, storytelling, field trips, programmed instruction, problem-solving, and demonstration. According to me, these are not the new methods but only the way to use these lectures in delivering the teaching. The innovation is based on evidence-based applications such as new approaches used in complying with the relevancy of the tool selected. The strategies should be used such that those students get motivated towards college who do not wish to study. Special considerations should be given to students who are economically weak and needs assistance. The inclusive settings like community areas, wards, and labs should be chosen to work with students. The teacher must think out some ways to critically empower students and be creative. Also, the teacher must think out some activities and techniques that are creative, fresh, and ready to use. Though, the nursing profession is such that it involves theoretical as well as practical training that needs to be integrated into the system (Öztürk et al., 2017).

My educator followed many strategies such as simulation, case-based learning, use of multimedia in the classroom, problem-based learning, e-learning, video conferencing and web-based conferencing, blackboard learning, or web-based learning and active classroom learning.

Simulation

The simulation took place in the way that it is a strategy that does not require amplification, anticipation, and an image to know real-life situations with guidance in an interactive manner. Although, this exercise involves attempting to a problem authentically with role play, skit, models, and standardized activities. The three-dimensional simulations like computer simulations, virtual reality simulations, and video interactions were used in the classroom by our professor. This method was used to practice and present the matter in an interactive manner (Solheim, Plathe & Eide, 2017).

Case-Based Learning

This approach was followed by our professor to give opportunities to students to give them more opportunities in learning and exploring the matter on complex matters and situations.

Problem-Based Learning

Problem-based learning is an instructional method in which students of my class actively participated to understand the concepts and develop problem-solving skills. It helped the students to critically apply the knowledge on problems and reinforcing motivation to life-long learning (Kaya & Öz, 2019).

Video Conferencing and Web-Based Learning

This method helped the students of my class to concentrate more and get connected to clear quarries. As my class was of students from diversified backgrounds and groups, this method helped in making more connections.

E-Learning

E-learning method helped students of my class in self-directed learning through distance learning methods on various advanced topics. This involved refocusing from educators on learning through the internet.

Tele Teaching

It is an online mode of teaching that involves direct interaction with a tutor. It promotes and involves discovery learning at home or office.

Blackboard Learning or Web-Based Learning

Our educator uses to ask us to learn from internet sources where they provide a space for face-to-face interaction. This tool enriches learning experience with the methods that enhance knowledge and skills to achieve organizational goals.

Hence, the combination of social media and technology has a crucial role to play in learning and promoting the integration of technologies, personalization, and humanization of virtual interactions. The new technologies should be emerged and bought together to reform and revitalize the education system. The emerging and innovative ways have been discussed in the paragraphs that support the view that technology increases effectiveness in improving learning and achievement. Moreover, I feel that the teachers must be fully trained and incentivized to use new technology. Many challenges will come while implementing these systems in the recent era due to the numerous demands of patients from the healthcare system. Teachers, as well as students, must be ready to contribute their best part for improvement in the education system. A critical thinking plan should be used so that it will help in achieving the objectives. The mechanism flows in the sequence of critical thinking gives rise to the imagination, imagination gives rise to plans, plans give rise to actions, and actions help in achieving objectives.

Learner development – the role of feedback approaches and adjustments for learning needs/support.

Feedback is the method that is proven to be very effective in bridging the gaps between assessment and learning. It has been proven that high-quality feedback improves the performance of students by eight months in the learning period. The same happened with me in the class and found a strong base behind the impact of feedback. I found that it is a cost-effective approach that enhances student's outcomes and helps during implementation in the education context.

In my nursing class, we use to deal with two types of assessments such as summative and formative. These assessments provided us with the opportunity to gather evidence and move towards progress. Our teacher used to give us this feedback on learning and provided us with the way to move forward. The results of assessments provide feedback to teachers by allowing them to evaluate and adapt strategies that ensure the students are meeting the standards of performance. The studies and reports proved that the students must be provided regular feedbacks on learning to assist educators and children. In this way, my classmates understood the use of feedback in learning. Based on my learning from nursing classes, I found that praise for intelligence upgrades learning. It is so because, whenever my educator said to me that "You did a brilliant job, you are very smart", it reinforced my mindset (Mosetlhe, 2018). 

Effective feedback was found by me as a way to determine the level of understanding and skill development by following the learning interventions and goals. The disabled students found the feedback a compelling influence on learner's achievement. It describes the path that what learners want to know and what do they understand in improving errors and misconceptions. However, feedback is found to be the incremental process that evaluates the assessment and learning. The ongoing and frequent feedback is significant to make improvements and achieve the goal. It involves understanding and performance delivery by practitioners, peers, and other learners. The effective feedback enhances the quality and strategies to make adjustments in progressive work of learning. The reporting to family and parents twice a year includes formal statements so that the students could improve on their weak points and assist them in improving performance.

I found that disabled and healthy people should determine the level of skill development and understanding to plan their way towards goal attainment. This way everyone gets evidence on skill development and knowledge enhancement. The progress measurement enables the practitioner to take steps according to plan for learning mechanism. Learners in this way confirm to strategies that they have to follow so that they can improve their knowledge. Many of the researches said that feedback should be of high quality so that they could affect the learner’s achievement.

The feedback that I use to get from my educator included the quality of work processes that I deliver during my practice sessions, motivation, and challenge overcoming tips to develop skills and knowledge, recognition for the work that was up to the mark. Hence, it directly links to success criteria and learning intentions. In my institution, our principal followed one more way to understand better that was feedback was given by learners to educators. This was given to show a close link between the work of students and the benchmark to reflect on the strategies that the educator should follow to make progress in learning. Also, the feedback was shared by peers, but that was an informal one. It was given to improve the productivity to concentrate on strategies that could build up everyone's success.

According to me, feedback begins with clarification on goals and activities that should be undertaken so that every process is demonstrated clearly. The intention of learning should be appropriate so that knowledge sharing and clarity of purpose are well known to everyone. The clarity should be there on skills set, understanding, and knowledge so that students could easily understand the focus of conducting this exercise. Some of my batch mates set the learning outcomes by themselves based on the effectiveness and success criteria constructed by partners. The useful model of feedback is meeting the progress as presented by three considerations such as 'Feed up, Feed Back, and Feed Forward'. It means that knowing about what are my learning outcomes, how am I progressing, and what should be the next step to make the performance better. This exercise should be done on time so that one must know the actions that could help him/ her in the improvement and monitor the efforts regularly (Burguete Dualde, 2018). The classroom activity should be done so that learners could participate in different levels of understanding. The educator also comes to know about what repetition should be done, and the different methods that will help more in discussion and practice so that learners can move forward.

Hence, from the discussion, it made me clearer that feedback provides transparent and focused strategies to help in improvement. The written and descriptive form of feedback refers to the preliminary discussion on goals and success criteria. The specific guidance on improvement enables the learners to think in detail on the specific matters that upgrades the successful completion of the task. It not only states the errors that require consideration of work and thinking but also the idea of why the performance lacks full understanding (Ahmad & Tawalbeh, 2015). The errors are fundamentals of improving feedback to move towards greater understanding and success. Belief and self-direction should be enabled by the student to complete their assigned tasks and goal attainment. Though, feedback is the grade that comments on performance and learning journey to avoid mistakes. Multiple forms of feedback are discussion, comments, and questions that encourage motivation and engagement to succeed. The educator must provide feedback on the assessment tips of learning, asking questions on a timely basis individually. It revolves around the concept that what is the area that needs improvement by an individual to offer advice on future steps of consideration. However, this two-way process supports learning and mutual respect for the continuum of exercises.

Identification of learning and development to meet business needs, program design, quality management, and evaluation and evidencing impact

Evaluation, measurement, and monitoring was the part of my organization to know the opportunities that could be there is a part of the organization's framework. The methods are arranged to know the practical guidance on evaluation and monitoring procedures. The measurement of actions is done to know the impact of those activities and provide guidance on training. The monitoring and evaluation is the process that is carried out to improve the performance and results. In the starting, the learning and development opportunities should be identified to have effective systems in place so that clear responsibilities are accessed.

According to me based on my learning, then consideration should be given to make development systems available with training. The training should consider the needs, performance, cost, delivery, and evaluation means. The e-learning packages that were available for us were developed with several ways that should be considered such as recommendations, reviews, learning outcomes, information, and referrals. The face to face training was provided to us by internal staff to sharpen our skills rather than external trainers. This procedure was followed to look at the course plan and have a look at the content that is to be used. The content that was shared was based on local practice and nursing procedures to be done in healthcare institutions. Moreover, the trainer was advised by the organization to share information that safeguards in leading the patient care. The nursing care was taken to overlook the whole process by maintaining the records and keep them updated. The identification was done to know the individuals responsible to manage the tasks.

While performing the activities, my educator uses to conduct a quality assurance session so that the students can actively participate and attend the sessions. Moreover, the guidance was given on the topics so that monitoring and evaluation should be done. These assessment models help the individual in promoting the internal support of members (Andrigue, Trindade & Amestoy, 2017).

After the learning and development activity was carried out, the follow-up session in my class was conducted to know the items critically. Several traditional methods were used to measure performance such as happy sheets. This is the robust method that considers the difference, extent of learning and development needs, objectives that are to be achieved, commitment, implementation, support of members, and the benefits. The learning, ongoing dissemination, and cascade of learning incorporate various forms such as supervision, team meetings, case discussions, observations of workplaces, and annual appraisals to commission the needs as per expectations for learning and putting into practice.

My educator framed some outputs to an activity so that the change in the education system could last longer. The outputs include development and improvement in learning interventions such as training. This includes briefing of notes and practising workshops to update the rules and regulations by conducting timely workshops. The ongoing assessments and reviews measure the strategies and business plan according to the benchmarks set. The challenges, discussions, and debates are the senior-level tasks that involve the collection of information from newsletters and websites. Evaluation and monitoring is the continuous process that impacts the learning process with appraisals and staff supervision by peer reviews, case meetings, and team involvement. The audits were conducted to incorporate the plans that inform future learning and development programs.

The measurement of impact was done by including a variety of information and data both quantitative and qualitative so that the evidence in patient care like referral rate, errors, recording, confidence, staff development, and positive outcomes presented in the form of audits for example. The programmer was run with the basis of learning outcomes and appropriateness of care. Case studies were given to demonstrate the journey of individual and measurement of impact in a powerful manner. The checklists were formed to consider the training with reasoning on the identification of matters, the workforce needed, and the respective roles of them. Budget is the major part that needs to be considered while implementing the systems and it should be such that the free cost of delegates is covered. Hence, the stage-wise procedures should be followed to agree on the processes of learning and development by volunteering and staff involvement. The needs should be first identified with the help of safeguarding opportunities and arrangements for promotion. The courses should be evaluated on the pre and post basis to upgrade the level of confidence and link the objectives with training that are to be provided. The generic evaluation should be done to know the venue, content, and delivery within the stipulated time. The educator plays a critical role in performance judgment and interaction within the members to make the changes if needed. The forms are the responsibility of the educator to comply with the processes. Similar to the management of my organization the individuals should be motivated to take forward the steps by providing training. The requirements are set to be met for safeguarding the patients and protecting the rights of organizations and individuals. The plans form the outcome clarification so that needs of the learning course are complying with the standards (Ayong & Atanga, 2017).
