Output 1:

To provide the education program to the parents regarding the importance of immunization and the effect of infectious disease over health status (Otsuka-Ono, Hori & Ohta, 2019). .

Parent awareness regarding immunization will be reviewed

Rate of immunization after educational program to predict effect of program.

The data will be collected from the health survey done by the government so that proper data can be used. The data will be collected after every six months to analyze the effect of the educational program over the awareness of the parent that directly affect the immunization rate.

The willingness of the parent for the education program can directly affect the goal of the intervention that directly decreases the risk of vaccine-preventable death.

Output 2:

The school-based immunization program will help to reduce the risk for vaccine-preventable deaths (Perman et al. 2017)

The attitude of school staff will be analyzed to understand effect of immunization over children

The school data be analyzed to predict the rate of immunization

The tally sheet can be used to provide accurate information regarding the dose of vaccine. The tally sheet helps the health care worker to maintain the record of the immunization that can be used to analyze the result associate with the program.

The poor weather condition can directly alter the visit of the health care professional for the vaccine that can directly affect the health status of the younger ones.

Output 3:

The informed consent of the patient regarding the vaccine schedule continuously monitor the vaccine safety (Griffith, 2016).

The partnership of the parent will be analyzed to understand effect over immunization

The schedule immunization will be analyzed to measure the progress of the immunization.

The immunization cards are one of the effective ways to maintain the immunization schedule. It will help the parents to maintain the proper immunization routine according to the immunization cards. 

The careless behavior of the parent can lead to miss in the schedule of the vaccine that directly misbalances the immunization schedule which can increase the risk of death.
